SkinDuo™ is a unique one-of-a-kind product developed by The BioArte, that is composed of 2 separate products that simultaneously target gastrointestinal health and skin homeostasis. The science behind SkinDuo™

Restoring the gut-skin axis

Gut-skin axis has been studied extensively, showing numerous skin diseases associated with an altered gut microbiome. The inner surface of the gut and the outer surface of the skin are both covered with epithelial cells, which directly contact microbes. The human intestinal epithelial barrier regulates the uptake of fluid, electrolytes, and nutrients, as well as prevents the penetration of pathogens.

Gastrointestinal dysfunction and imbalance of the intestinal microbiome, known as dysbiosis, cause excess bacterial growth of pathobionts, leading to competition for nutrients and toxic metabolite production. This results in local and systemic inflammation, affecting the physiology of the skin. Maintaining the integrity of tight junctions and the permeability of intestinal epithelial cells is crucial.

Commensal bacterium Propionibacterium acnes IA acts as an opportunistic pathogen in Acne vulgaris. The colonization of the pilosebaceous follicle by P. acnes is considered one of the central factors affecting the pathogenesis of acne. Another two factors involved in acne are increased sebum production, with a change in its composition, and an increase in the inflammatory rate. Our research has also explored the correlation between C.acnes and S.epidermidis, both present in acne-prone skin.

Looking at the gut-skin axis, it has been demonstrated that probiotics enhance the epithelial barrier, hence we evaluated the potential of the probiotic strains Bifidobacterium lactis BS01, Lactobacillus rhamnosus LR05, and Lactobacillus plantarum LP01 to protect the gut epithelium membrane against pro-inflammatory factors such as TNF-alpha + IL-1beta, as well as to restore the membrane integrity after damage, through a model of gut cells (Caco-2 cells) in Trans Epithelial Electrical Resistance (TEER) assays.

All the selected strains were very significant (p<0,001) both in the prevention of damage and restoring integrity after the damage had been caused.

After extensive research, we focused on the use of a single strain of Lactobacillus species for the treatment of acne, to utilize the beneficial modulation of the cutaneous microbial interactions in subjects with mild-to-moderate acne. The novelty of this formulation relies on the fact that the Lactobacillus species are alive. Viability is essential for the Lactobacillus species to suppress the colonization of pathogenic microbes and balance out the skin with health-associated commensal bacteria.

SkinDuo™ topical treatment has proved its ability to decrease local inflammation, by inhibiting the production of IL1alpha, IL1beta, IL6, and Il8, even in presence of pro-inflammatory molecules like LPS.

It is even able to drastically reduce sebum production from human sebocytes and compete with C. acnes IA for the colonization of hair follicles.

Acne severity has been shown to be associated with a reduced diversity of C. acnes strains compared to healthy individuals. It was found that among several C. acnes strains isolated from the skin of healthy individuals and patients with varying degrees of acne or other infectious diseases, IA strains were strongly associated with moderate to severe acne, while other strains IB, II, and III were associated with intact skin and opportunistic deep tissue infections. The IA phylotype is also preferentially increased in the inflammatory microenvironment, indicating a change in the skin microbiota of acne patients. SkinDuo™ has been shown to reduce C. acnes IA in human sebocyte models without altering the presence of S. epidermidis. Because commonly used topical and systemic antibiotics cause skin dysbiosis, our new understanding of acne pathophysiology has led to a change in acne treatment. Our concept is based on the development of a formula for acne-prone skin that makes it possible to target pathogenic strains, leaving intact commensal strains. Such alternative therapies, which involve changes in the microbiome, are the first evidence of “ecobiological” anti-inflammatory therapies.

What is sebum?

Sebum is the medical name for skin oil. It is produced by glands attached to skin pores all over the body. These glands are especially large on the face and upper body, where acne usually occurs. Acne cannot occur without sebum. Excess sebum production can lead to an overgrowth of skin cells that clog pores. When pores become clogged, sebum is trapped, providing a breeding ground for acne bacteria. In general, the more sebum you have, the more likely you are to develop acne. Studies show that an increase in male sex hormones in both men and women leads to an overproduction of sebum.

The type of fatty molecule found in fat is called a free fatty acid. Researchers discovered unusually high levels of fatty acids in clogged pores (comedones), suggesting that increased fatty acids in sebum may play a role in clogging pores. In particular, researchers have shown that fatty acids trigger a certain inflammatory substance in the body called interleukin-1 (IL-1), which is known to trigger acne.
